Day 1

Two doors open

Food stops: the energy sensor AMPK rises, mTOR goes quiet, and the front door of autophagy — the ULK1 cleanup program — swings open. This is the door water fasting uses too.

Water stops: your blood starts to concentrate, creating an osmotic pull on every cell. The cell registers a different alarm — not “I'm starving” but “I'm being squeezed.” That is hyperosmotic stress: a second, separate cleanup pathway that switches on even when nutrients are present. Day 2

The electrolyte fork — and the heat

Aldosterone surges and the body starts saving sodium while sacrificing potassium — in the 5-day clinical study, the divergence begins right around day 2. Stress hormones climb steeply across the fast to keep blood volume and pressure compensated.

And you feel warm. Sweat shuts off, blood pulls inward toward vital organs, and metabolic heat loses its exit routes. The famous dry-fasting heat is trapped heat, not “cells turning into furnaces” — expected, and normal.

Day 3

Deep cleanup

Inside squeezed cells, the microtubule rail network is dismantled and rebuilt. Dynein motor proteins haul the cleanup compartments into tight clusters around the cell's recycling center — a concentrated, organized cleanup instead of a diffuse one.

The deep-cleanup markers rise: LC3-II climbs, long-lived proteins break down faster, and the p62 protein aggregates that ordinary autophagy leaves sitting around get cleared. Meanwhile, the study's fasters keep normal blood pressure, heart rate and oxygen saturation — all three classic risks compensated by the body.

Days 4–5

The reservoir

This is where depth pays. The deeper pathway reaches what the mechanism article calls the tissue reservoir — where viruses like EBV hide from a shallow cleanup.

In the 5-day study, kidney markers stay in safe ranges and glomerular filtration actually rises substantially. The authors' conclusion: short-term antioxidant, anti-ischemic, immune-stimulating, anti-edematous and anti-inflammatory effects.

The refeed

Where the real magic happens

How you come back matters as much as the fast. Carbohydrates first: they refill glycogen fastest and trigger the insulin response that restarts your metabolism — a fat-first refeed pushes storage, not restoration.

And hold the salt at first: aldosterone is still high, and sodium straight after a dry fast invites edema. The club's practice for extended fasts: coconut water, fruit compote, vegetable broth.

Day 30
